If you're wondering why it is a "4th hand" and not a "3rd hand", a 3rd hand tool is the tool that holds the caliper arms closed - it is a sort of metal bracket, nothing like this tool which actually pulls the inner wire taut, allowing you to tighten the cable anchor bolt. It is one of those tools that is not necessary to adjust brakes until you find yourself performing maintenance on bicycles on a regular basis - then you'll wonder why you went so long without it.
